Thought I'd solve the puzzle now as I may not be online when this challenge finishes. Granma M sort of got it, the write up for this instrument is " Vintage dental extraction tool, the hook is placed under the roots of the tooth, the operator then pulls to remove said tooth" How the hook is placed under the tooth I really don't want to know!!!!!
