Thought I'd solve the puzzle now as I may not be online when this challenge finishes.
Granma M sort of got it, the write up for this instrument is " Vintage dental extraction tool, the hook is placed under the roots of the tooth, the operator then pulls to remove said tooth"
How the hook is placed under the tooth I really don't want to know!!!!!

I have not a clue unless it is a very very very old-fashioned tool for a doctor to look into a person's ear but I am looking "upward" on my tablet for the rest of the night so I don't have a "clear" view of the object. Great "screaming" painting and the cogs and the hinges which should give me a clue to the identity of this object but not yet. Great page for the challenge Beryl!!!
