I made the staples using the gradient tool and the blur tool and the journal shape by using a shape, a solid color layer, and the 'group with previous' tool. I extractected the picture using the background eraser, used the blur tool on the ribbon and pendant and touched up a copy of the paper clip to make it look like it is really cliped to the paper.
